2. Medical Services Provided
We provide women’s health and wellness services through telehealth and in-person consultations as necessary, including:
Women's hormone health and wellness
Comprehensive lab testing as needed
Sexual and reproductive healthcare (including contraception)
Well-woman care, annual exams
Perimenopause and menopause care, AND MORE…
Limitations of Services:
No emergency care: If you have a medical emergency, call 911. If you are experiencing a mental health crisis, call 988.
Not available 24/7: As a private direct care practice, the provider is not on call outside of office hours.
3. Appointments and Scheduling
Appointments must be scheduled online. If you experience issues, you may call our office.
New patients: Must complete and sign all required forms before an appointment is confirmed.
Cancellations & No-Shows: You must cancel at least 24 hours in advance to avoid a $25 fee. After three no-shows, you may be dismissed from the practice.
4. Telehealth Services
We offer telehealth consultations as part of our services.
Limitations: You must have a Primary Care Provider (PCP) for routine medical care and ensure that you are up to date on screenings.
5. Payments and Billing
Direct payment only: We do not accept insurance but provide a Superbill upon request for potential reimbursement.
Accepted payment methods: Credit cards, debit cards, HSA/FSA cards, Zelle transfers.
No refunds: Consultation fees are for medical discussions and do not guarantee a specific outcome.
Payment due at time of service: Unpaid balances may result in appointment cancellation.
Insurance Disclaimer:
We do not accept insurance for visits but will collect your insurance information to submit for lab work, imaging orders, referrals, and prescriptions under your insurance if applicable.
You are responsible for verifying your insurance coverage for services.
6. Patient Responsibilities
By receiving care at Focus Women's Health and Wellness PLLC, you agree to:
Provide accurate medical history and health information.
Follow your treatment plan as discussed.
Respectful communication: No abusive language or harassment will be tolerated.
Notify us of your preferred name and pronouns, if applicable.
Failure to adhere to these expectations may result in dismissal from the practice.
7. Privacy and Confidentiality
We comply with HIPAA regulations and safeguard all patient records:
Storage: Patient records are stored electronically in Charm EHR (HIPAA-compliant system).
Encryption: Documents emailed to our official email are end-to-end encrypted.
