Appointment request
Completing the steps below sends an appointment request. Before approval, you'll need to complete and sign the required forms. Questionnaires and consent forms must be submitted at least 24 hours before your appointment. Your completed questionnaires help determine the level of care required and associated fees. Please fill them out accurately.
For perimenopause/hormone health care, consider a Telehealth consultation unless a physical exam is necessary. Telehealth is cost-effective for you and helps the practice run more efficiently, with lab orders sent to a nearby lab and results reviewed electronically. Treatments are sent directly to your pharmacy. In-person visits are best for those without a primary care/OBGYN provider needing an annual exam or with symptoms requiring examination. Telehealth visits, including lab orders or prescriptions, are available only to Texas residents. Those outside Texas can request a health coaching session below. Thank you for your cooperation!
